I understand I must provide to the Apitipi Anicinpek Nation Education Department all relevant information requested about me, my enrollment and my grades from the institution I will be attending. I further understand must complete and sign all relevant documents enclosed in this application package and provide the additional information needed to become a successful applicant for Post-Secondary Sponsorship Program.
I understand I am to be enrolled in sufficient courses to be considered a full-time student of the Institution I am attending. I also understand I must advise the Apitipi Anicinpek Nation Education Department of any changes in a course, or program, I may be required to make during any of the semesters I am required to complete.
Should I encounter any difficulties during any semester, I understand only I am responsible to take the immediate action of seeking assistance from a Student Counsellor of the Institution I am attending. I also understand I must collaborate with the Student Counsellor to advise the Apitipi Anicinpek Nation Education Department of any changes in a course, in a program or of any tutoring I may need to address any academic difficulties I may encounter.
I understand I must advise and request for authorization from Apitipi Anicinpek Nation Education Department of my decisions to withdraw from a course, or a program, before the end of the semester. I understand my action to withdraw form a course, or program before the end of a semester, without authorization from the Apitipi Anicinpek Nation Education Department, may render me ineligible to receive sponsorship for at least one complete academic year and I may be required to reimburse in full, or in part, the sponsorship I received for that incomplete year.
As a sponsored post-secondary student of Apitipi Anicinpek Nation, I understand the importance of representing my community in demonstrating the utmost respect and consideration for fellow students, personnel and property of the institution I am a student of. I understand and agree Apitipi Anicinpek Nation Education Department shall not be held responsible or liable for my behavior and/or conduct if I commit an indictable offense while on, or toward the property of the Institution I am a student of.
I understand I am to attend classes on a regular, if otherwise, required, daily basis and to hand in completed assignment on time. I understand poor attendance and missed assignments may render my monthly living allowance to be withheld.
I understand I am to provide Apitipi Anicinpek Nation Education Department copies of my mid-term and final report/marks, and failure to do so may result in my living allowance to be withheld.
I understand and agree my monthly living allowance is intended for the purposes of paying shelter costs, such as room and board, or rent and for other living expenses. I further understand the living allowance portion shall be issued on the 24th day of each month. Should the 24th fall on a weekend, I understand the living allowance shall be issued the next business day.
I understand I am obligated to report, if applicable, my spouse or any dependents who reside in the same household as me. I also understand that I am not to claim for, my spouse or any of the dependents that may have already been claimed by me or my spouse, under any other Post-Secondary Sponsorship Program.
I understand the Apitipi Anicinpek Nation Education Department does not provide any pay advances under its Post-Secondary Sponsorship Program.
I understand the Apitipi Anicinpek Nation Education Department and the Apitipi Anicinpek Nation Band will not be responsible for any loans (OSAP, Bank Loans, Personal Loans) incurred by me during my studying.
I understand my living allowance is only based on my full-time enrollment in a college/university/online studies. (i.e. 4 credits per semester = 12hr or more of instructional time. Anything less of 4 credits per semester or less than 12hrs is part-time).

By submitting this form, I am declaring to be in agreement to the conditions outlined in this document.
